Introduction
As rapid aging progresses, many patients face multiple complex medical issues rather than a single disease, increasing the need for integrated care for patients with severe, complex conditions.
To improve the training environment for residents, the Ministry of Health and Welfare initiated a pilot project for hospitalists in September 2016, aiming to minimize personnel gaps in medical institutions caused by limited working hours and the absence of residents in some departments.
As a result of participating in this pilot project, our institution has established and is operating a Department of Hospital Medicine.
The Department of Hospital Medicine operates under the mission of "Revolutionizing inpatient care," composed of specialists in hospital medicine, intensive care, and rapid response teams (INHART). It is a new integrated care system that focuses exclusively on inpatient care and takes direct responsibility for patient treatment.
We have established a system that allows for a multidisciplinary approach within the Department of Hospital Medicine, ensuring no treatment discontinuity with existing departments through organic cooperation and integrated treatment for patients.
The Department of Hospital Medicine provides "patient-centered comprehensive optimal care" for hospitalized patients, enhancing the quality of medical services and patient safety, thereby ensuring patient satisfaction.
